When reporting/registering the death of a Filipino citizen, the following documents should be submitted:

  1. Duly accomplished Report of Death (4 original)
  2. Original and 4 photocopies of the following:
    • Death Certificate issued by the Australian Registrar of Births, Deaths and Marriages;
    • Photocopy of Data page of the Philippine passport of the deceased;
    • Mortuary Certificate* issued by the Philippine Consulate General (for shipment of human remains or urns for repatriation to the Philippines)
  3. Processing fee of A$45.00 in postal money order made payable to the Philippine Consulate General. 
  4. Return Registered envelope, person to person (blue sticker) or Express Post envelope, signature on delivery for the return of ROD.

RELEASING

  • The personal copy of the ROD will be sent by mail the next working day from receipt of complete documents 
  • The Consulate assumes no responsibility for any delay, loss or damage in the mail or while the documents are in custody in courier service.

The Philippine Consulate General will transmit the ROD to the Philippine Statistics Authority (PSA). There is a waiting period of at least 4 to 6 months for the availability of PSA copy in security paper. Please get it from PSA by providing the transmittal details (Despatch number, Reference Number and Date of transmittal).

The PSA copy will initially be available at PSA, Solid Bldg., Magsaysay Blvd., Sta. Mesa, Manila.
